Currently seeking LPN's or RN's in the Lansing area to work within an office setting and perform Telephonic patient care. The main responsibilities include speaking with patients over the phone, provide health risk assessments and communicate effectively with patients, providers, and caregivers to provide health education to patients. Must haves include: LPN or RN license, Long Term Care experience, Home Health Care experience, education to patients with chronic illnesses, Disease Management, Case Management, and at least an Associates degree.
